If you are insured by Aetna, CVS runs the whole show it appears. Aetna has 11% of the US health insuraance market. Their practices may be (probably are) illegal due to the vertical integration, but to call them a "monopoly," under US law, I believe you have to have a far larger market share.

 The Rockerfeller's Standard Oil's  control of the US oil industry, through vertical and horizontal integration, was 80 or 90% of the US market.

And you have not explained how it would work without Walgreens and CVS. Walmart is in the mix too. They are the three largest pharmacies in the US (which, in and of itself, proves there is no "monopoly"). Rite Aid is restructuring, so it is not doing well.
