You think no one  at HHS raised an eyebrow when they included "safe smoking kits" in the language ? You think no one at HHS had the oresence of mind to google "safe smoking kits" when they wrote the grant ? You think no one at HHS had an inkling of what may be included in a "safe smoking kit" ?
The " source" is the grant language.

Maybe the WFB reporter got things wrong about the contents of the safe smoking kits.  Maybe the reporter never asked about crack pipes.  Maybe he just assumed the smoking kits would include the pipes because a few cities have experimented with their distribution.

There’s an old adage in journalism, “If your mother says she loves you, check on it.”

My guess is the grant program staff did not flag the crack pipe issue to HHS senior management, and therefore it was not brought to the attention of the White House.   

That’s not good, but it’s something that can be fixed.  Future grant solicitations should include a definitions section.   And senior management should be made aware of any significant policy issues.
